<_10/> <_0/> General Information This form is to be used in Data Quality Assessments in health facilities in the country Background Reports on health system strengthening activities are only useful in making policy decisions and improving health care services if the data contained in the reports are of high quality. Attention to data quality ensures that target-setting and results reporting are informed by valid and reliable information, and that reporting officers, units, departments, agencies, programmes and partners are collecting and organizing this information in the same manner. Since the inception of the revised NHMIS (Nigeria DHIS2) in 2013, much emphasis has been placed on assuring the quality of routine data reported from health facilities to the national level for health programmes planning, monitoring and evaluation. Whether for the National Health Management Information Systems (NHMIS) or for donor-funded projects like PEPFAR, PMI and the Global Fund, data quality assessment tools and methods have been developed and widely used to measure data quality (in terms of accuracy, completeness, consistency and timeliness of data) and identify gaps in reporting systems which inhibit the production of quality data. Existing tools used to measure data quality and identify gaps in reporting were harmonized with the NHMIS DQA checklist, including WHO Immunization Data Quality Audit (IDQA) and Data Quality Self-Assessment (DQS) for immunization, the Global Fund/MEASURE Evaluation Data Quality Audit (DQA) for HIV, TB and Malaria indicators, the Routine Data Quality Assessment Tool, an indicator-generic capacity building and self-assessment version of the DQA, and the PRISM methodology which measures data quality (as a component of HMIS performance) in a similar fashion. This will reduce the ad hoc implementation of these tools in-country by different disease programs and donors which often produced overlapping and redundant assessments which add burden for health workers and is wasteful of precious public health resources. Data quality assessment (DQA) is an institutional process that involves regular review of NHMIS performance as measured by the level of data quality and the use of data for decision making. Regular data records review and periodic DQA exercise are necessary requirements for a functional and effective National Health Management Information System (NHMIS). The DQA exercise is designed to consistently ensure the quality of NHMIS data at all data management levels. Data quality assessments help improve data quality by uncovering hidden problems in data collection, aggregation, and transmission of priority indicator data. DQA should be conducted regularly and linked to the annual health sector review and the quinquennial health sector planning cycle. To build technical capacities for DQA, synergies would be enhanced by the Ministry of Health with technical/development partners, statistics agencies and academic institutions. Development and technical partners can greatly contribute to the success of NHMIS data quality plan and mechanisms. DQA should be conducted in a spirit of openness and transparency and should include regular feedback to data producers especially at the LGAs and health facility level.
